Once a case is filed with the Court, Pursuant to Minn. R. Gen. Prac. 104, the parties are required to file a Civil Cover Sheet (formally known as an Informational Statement) The form and information to be included in the Civil Cover sheet is set forth in Minn. R. Gen. Prac. 111.02. There is a very specific format for this Civil Cover Sheet. After all parties have submitted Civil Cover Sheets, the Court should issue a scheduling order.
